If you are interested in being seen by Dr. Stout or Shane Gahn, please fill out a new patient intake form below. This is the quickest way to get started. Once you have filled out our initial forms, we will get you scheduled.

Currently we are accepting new patients and are scheduling patients around 2-4 weeks after the initial form is filled out.
